CHS DANCE 
Middle School Dance Intensive & Concert Information
Grades: 6th-8th

Registration Fee: $40-Includes a Dance Team T-Shirt, Master Class & a student ticket to the Dance Concert to watch- after they perform.

Steps For Registration:

1. Click Here to fill out the registration google form.
2. Print & sign the LVUSD hold harmless waiver-Click Here    Then mail in with payment or if paying by Venmo please email form to  jbogartz@hotmail
3. Pay $40 using one of the methods below:

           ** Venmo @Jill-Schwartz-14
           Please include student’s name, grade & “CHS Dance                             Clinic” 
           **Or mail a $40 check written out to CHS Dance: 
           Calabasas High School Dance
           c/o Kristen Furino/CHS Dance Clinic
           22855 Mulholland Hwy, Calabasas, CA 91302
